TMJ disorder is usually a manageable chronic condition. But there are moments when TMJ-related symptoms become acute enough to warrant urgent attention. Knowing the difference between normal TMJ discomfort and a situation that needs immediate care helps you respond appropriately. At Magnolia Way Dentistry in Apex, NC, Dr. John Wayand evaluates and treats TMJ-related pain and can help you determine the right course of action.
7 Signs Your TMJ Symptoms Need Prompt Attention
1. Your Jaw Has Locked Open or Shut
Jaw locking — where the joint seizes and you can’t fully open or close your mouth — is one of the clearest signs that something has mechanically gone wrong. The articular disc inside the TMJ can displace out of position, causing the joint to catch or lock. If your jaw locks and doesn’t release within a few minutes, or if this happens repeatedly, don’t wait to seek care.
2. Sudden Severe Pain That Wasn’t There Before
Many TMJ patients have a baseline of mild, manageable discomfort. A sudden spike in pain — significantly worse than typical — can indicate an acute joint injury, disc displacement, or an inflammatory flare requiring intervention. Pain that appears abruptly without an obvious trigger is worth evaluating promptly.
3. You Can’t Fully Open Your Mouth
Normal maximum jaw opening is approximately 40-50mm. If your opening is suddenly and significantly restricted — to the point where eating or speaking is difficult — this indicates significant joint or muscle dysfunction that warrants evaluation.
4. Clicking Has Been Replaced by Grinding or Crunching
Mild clicking when opening or closing is common and often harmless. A change to a grinding, grating, or crunching sound can indicate that cartilage in the joint has deteriorated and bone-on-bone contact is occurring.
5. Pain Is Radiating to Your Ear, Neck, or Causing Headaches
When facial and jaw pain becomes severe enough to significantly impact sleep or daily function, the priority should shift from self-management to professional evaluation.
6. Swelling Around the Joint
Visible or palpable swelling in the area just in front of the ear can indicate acute inflammation, joint injury, or, in rare cases, infection. Swelling that appears quickly or is accompanied by fever needs prompt evaluation.
7. Symptoms Appeared After a Jaw Injury
If your TMJ symptoms developed or significantly worsened following a blow to the jaw, face, or chin, the joint may have been directly traumatized. Post-injury TMJ problems benefit from early evaluation to prevent the injury from setting in a way that’s harder to treat later.
What Not to Do During a TMJ Flare
- Don’t force your jaw open if it’s locked — apply gentle pressure only and seek help if it doesn’t release
- Avoid hard, chewy, or large-bite foods
- Don’t chew gum
- Avoid extremes of jaw movement — wide yawning, lengthy talking
